A tiny should always be very clear on Who is in charge. By holding the tiny, Carla is showing just how helpless he is. he cannot escape unless She allows it. If She chooses, She could crush him to a pulp with the merest twitch of Her thumb. All that is sufficient is a little squeeze. he hasn't done anything wrong, but the pain reminds him Who is boss.
